Paradigm Health is expanding rapidly throughout Indiana and we are opening several new positions.*Position Details:** Full-time Hospice RN Case Manager position* Schedule: Monday thru Friday, 8a-5p* Available territory openings: Madison and Delaware Counties* RN Case Manager typically assigned 1 county to cover.*Benefits:** Competitive salary* Mileage reimbursement* Excellent PTO program (up to 136 hours in first year)* Every Friday is payday!* Health, dental, vision* 401k match program* Great team environment!*Job Functions:** Performs physical and psychosocial assessment on all hospice patients.* Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.* Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.* Assesses patients condition, initiates plan of care, re-evaluates and updates as necessary.* Obtains all necessary physician orders and collaborates with physician as patients needs dictate.* Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.*Requirements**Experience:** One year Hospice experience preferred, but not necessary* 1 year of RN experience required*Credentials:** Licensed Registered Nurse in State of Indiana* Current driver's license* Must have reliable transportation and insurance*Statement:*It is the policy of Paradigm Health to provide equal employment to its employees and qualified applicants for employment regardless of race, color, sex, religion, national origin, veteran status, or physical or mental disability, or as otherwise required by an applicable local or state law.
