Community Outreach AssociateAspire Indiana Health is a nonprofit
provider of comprehensive "whole health" services including primary
medical care, behavioral health, recovery services and programs
addressing the social determinants of health such as housing and
employment. Aspire has health centers in five Central Indiana
counties serving Hoosiers of all ages and walks of life.Position
SummaryThis frontline position is centered on a philosophy of peer
support and must be a trusted member of the community. This person
must have a deep understanding of the vulnerable populations we
serve, including the economically disadvantaged, racial and ethnic
minorities, the uninsured, the homeless, those with HIV, and other
chronic health conditions such as substance use disorder and mental
illness. The Community Outreach Associate will be responsible for
identifying, educating, and assisting individuals in various care
coordination activities. Activities will vary based on individual
needs, but the primary function is to help potential patients find
a medical home at one of Aspire's Federally Qualified Health
Centers (FQHC). The goal of this position is to improve the health
of the community by increasing health knowledge and
self-sufficiency through outreach, community education, informal
counseling, social support and
advocacy.Education/Experience/RequirementsHigh School diploma
required; Associate's degree in a related field preferredPrevious
experience with non-profit behavioral health, substance use or
healthcare highly preferred. Proven experience and connections with
local community, service providers, etc., highly
preferredIntermediate to advanced computer skills required.Must
possess/maintain a valid driver's license, current automobile
insurance and a driving record that meets the guidelines and
requirements of the organization. Must also have reliable
transportation to transport self and/or others and to attend
